Enrolment Process

There are 4 STAGES to enrolling your child at Lakelands Primary School. These are;

STAGE 1 – DOWNLOAD

2024 ENROLMENT (current year)

Form 1 Student Health Care Summary– This form is required for all new enrolments together with the Application for Enrolment From

2025 ENROLMENT (next year)

Form 1 Student Health Care Summary– This form is required for all new enrolments together with the Application for Enrolment Form

**The Application documents must be completed and returned to the school along with:**

Documents can be scanned and emailed. Lakelands.PS@education.wa.edu.au (PLEASE NOTE: 10MB limit on emails)

STAGE 2

Meeting with the Principal or Deputy Principal
Enrolment Form to be completed

STAGE 3

Complete Enrolment Form and return to the school
Payment of School Voluntary Contributions and Charges – Click

Purchase School Uniform – Click

Purchase Personal Items – Click

STAGE 4

Letter of confirmation (PP – Yr6) and faction allocation when student commences.
